If you are travelling abroad please complete the Travel Assessment Form below 6-8 weeks prior to travelling. A travel vaccination nurse will asses the form in due course and then contact you via telephone to discuss your travel needs.
Any travel risk assessments submitted with a departure date of less than 6 weeks away will not be actioned by our Travel Nurse.
If your departure date is less than 6 weeks away, you will need to contact a Private travel clinic for further assistance.
Travel Assessment Form
We no longer provide a full travel risk assessment; we will only offer the following vaccinations if necessary: HEP A, Typhoid & Tetanus, Polio & Diptheria. The Travel Nurse will book this appointment for you.
If you are signed up for the NHS App you can view your current immunisation history. For all other travel advice including what vaccinations are advised, you will need to attend your local private travel clinic. Please note this is a private service and you will be charged accordingly.
The following websites will help you:
Masta Travel – “We offer travel health consultations, vaccinations, anti-malarials and travel related retail items, as well as a wide range of non-travel vaccination services.”
Fit For Travel – “It gives travel health information for people travelling abroad from the UK.”
