How to Request a Home Visit

We provide a home visiting service for people who are housebound as a result of frailty or illness. We would ask you to try and phone the surgery on 01228 588121 prior to 10.30am if you expect a home visit to be necessary to help us to plan the workload for the day. Visits are prioritised based on clinical need.

We would encourage everyone to try and attend one of our surgery sites when they need to access our services.

In addition, we work closely with our community and social care colleagues to help co-ordinate care for some of our most frail housebound population. This includes a large degree of planned and proactive care planning to try and support people to live well with their long term conditions/frailty and to help people to prepare for their future needs
